Vision Coverage

Vision offered through Anthem presenting access to eye exams and lower costs for glasses and contact lenses.  Eligibility begins date of hire.

Company paid benefit provided through Lincoln Financial Group.
 

Paid Time Off

An annual allotment for Corporate Employees only that is available January 1st each year to be used for all absences due to vacation, personal time or illness. New hires receive 15 days of PTO a year prorated based on start date. The number of days increases 5 days on January 1st after 5, 10, 15, years of employment with the company. Field Employees do not have an allotment of PTO as they are on call 24/7/365. Field Employees may take as much time off as they wish with the understanding that they take calls or have backups aligned to cover their district.
 

Health Spending Account

A tax-advantaged medical savings account available to employees enrolled in the high-deductible health plan. Company makes a monthly deposit on behalf of enrolled employees each month to the employees account. Funds deposited roll over and accumulate year to year if not spent. HSAs are owned by the individual and may be used to pay for qualified medical expenses at any time without federal tax liability or penalty.
 

Flexible Spending Account

A tax-advantaged account that can be set up by employees enrolled in the PPO plan. Allows employees to set aside a portion of earnings to pay for qualified medical expenses not covered by the plan (medical, dental or vision) or for dependent care. Money deducted from an employee's pay into an FSA is not subject to payroll taxes, resulting in substantial payroll tax savings.
 

Short-Term Disability

Company paid benefit providing income protection to benefit eligible employees who are unable to work for medical reasons that are not work related. Eligibility begins after 6 months of continuous service paying 60% of gross weekly salary to a mx of $2,500 a week after 5 day elimination period. Benefit increases based upon tenure with Company.
 

Long-Term Disability

Company paid benefit providing long-term income protection to benefit eligible employees who are unable to work for medical reasons that are not work related. Eligibility begins after completion of a 90 day wait period paying 60% of gross weekly salary up to a maximum of $5,000. Coverage provided through Lincoln Financial Group.
 

Life Insurance

$25,000 of Company paid life insurance. Employee may elect to increase this amount via payroll deduction. Coverage provided through Lincoln Financial Group.
 

401K

Dollar for dollar company match up to 4% of base salary. Eligible after 60 days. No vesting period for company match.
 

Dental Coverage

Dental offered through Anthem offering preventive and restorative services.  Eligibility begins date of hire.

Medical Coverage

Comprehensive medical plan through Anthem. Traditional PPO and HSA Plans with monthly Company contributions to HSA. Eligibility begins date of hire.
